Each eligible family will receive the Ohio Direction Card to be used to purchase food at participating stores.

Medicaid is a federal and state funded health program designed to provide medical coverage to low income individuals who are age 65 or older; blind, disabled; pregnant; children under age 21; and members of families who meet all eligibility criteria and the population eligible under the Affordable Care Act.  The determination of health program eligibility requires the completion of an application.  This program does not require an interview and resources are not considered. Minimal verification is required including income. An application can be obtained by calling 1-800-324-8680

Failure to fulfill the requirements of the SSC will result in loss of benefits to the family.  You may apply online at:  http://ODJFSBENEFITS.OHIO.GOV
